Target Population: Adults aged 40-65
Adults Requiring Psychiatric Support:
Support includes emotional care, consistent mental/physical health monitoring, support with medication and assistance during anxiety or depressive episodes.
Structured daily routines and support in community engagement are emphasized.
Adults with Mental Health Issues needing support in Accessing the Community:
Assistance with social interaction, outdoor activities, and cultural or religious practices to foster meaningful connections and enhance community participation.
Post-Hospitalisation Care:
Transitioning from Hospital Care: Continued medication, mobility, and emotional reassurance support. Development of structured daily routines to facilitate a smooth transition.
Mental Health Conditions:
Severe and Chronic Mental Disorders: Ongoing care to support emotional stability, crisis management, and long-term health monitoring.
Alcohol-Induced Dementia: Tailored cognitive and emotional care, with attention to safety and well-being.
Dual Diagnosis (Mental Health and Substance Abuse): Comprehensive risk management plans and support for dual conditions, ensuring consistent therapy and safety.
Residents Subject to Deprivation of Liberty Safeguards (DoLS): Regular capacity assessments and safeguards for autonomy within legal frameworks.
Daily Living Support:
Personal Care: Support with hygiene, grooming, and maintaining personal health.
Activities of Daily Living: Assistance with meal preparation, household tasks, and laundry to sustain a clean and organised living environment.
Social and Recreational Activities: Engagement in hobbies, group activities, and skill-building to enhance independence and well-being.
Community Access: Supervise and facilitate outdoor activities, appointments, and social interactions.
Family & Friend communication: Support maintaining relationships with family & friends.
Budgeting & Finance: Support managing finances.
Emotional & Challenging Behaviour: Support with emotions, anxiety and challenging behaviour.

While we provide personal care and support, this home may not be suitable for residents requiring dedicated 1:1 assistance with incontinence management or significant mobility needs. For those with these specific needs, our other homes, Evelyn or William House, may offer a more suitable environment with the necessary level of care.
